This information is used for I-9 employment verification and your driver qualification file.

Your CDL
Commercial Driver's License info. Take a clear photo of the front of your CDL — we'll verify the number and expiration with your state.
Held a CDL in another state before? Pro Logistics requires a minimum of 2 years of OTR experience. If your current CDL is less than 2 years old, list your prior state CDLs so we can verify your full driving history.

Your medical card
DOT Medical Examiner's Certificate. As of January 2026, electronic verification via the National Registry is the standard. Take a photo of your medical card too.

Where you've lived (last 3 years)
Every address you've called home for the past 3 years. Required by FMCSA 49 CFR 391.21.
Your current address from Step 2 counts as the first one. Add any prior addresses below.

Accidents (last 3 years)
Any motor vehicle accident in the last 3 years, whether you were at fault or not.

Traffic violations (last 3 years)
All motor vehicle violations you were convicted of (or forfeited bond for) in the last 3 years. Parking tickets are excluded.

Your last 10 years of employment
This is the big one. FMCSA requires 10 years of employment history for CDL drivers. List every job in order from most recent to oldest. Gaps over 1 month need an explanation.
If you weren't a driver the whole time, that's OK — list every job, including non-driving work. We're allowed to ask. Be honest, gaps are normal.

Drug & alcohol history
Required by Federal Motor Carrier Safety Regulations (49 CFR Part 382). Answer honestly — we verify everything through the FMCSA Clearinghouse.
By moving to the next step, you consent to:
A pre-employment drug & alcohol test as a condition of hiring, plus random, reasonable-suspicion, and post-accident testing during your employment, in accordance with 49 CFR Part 382.

FMCSA Clearinghouse consent
Federal law requires Pro Logistics to run a full query of your record in the FMCSA Drug & Alcohol Clearinghouse before hiring. We need your written consent for that.
Why this matters: The Clearinghouse is a federal database that tracks any positive drug/alcohol tests, refusals, or violations from your past employers. Without your consent, federal law prohibits us from hiring you for any safety-sensitive role.
I, the undersigned, voluntarily and knowingly authorize Pro Logistics Services Inc to conduct a full query of my record in the FMCSA Drug & Alcohol Clearinghouse, as required by 49 CFR §382.701(a). This consent is effective for the duration of my employment.

Hours of Service Policy. I will comply with all FMCSA Hours of Service regulations. My logs are recorded automatically by the company-provided ELD (Optima) — I am responsible for keeping my ELD account current, certifying my logs daily, and reporting any malfunctions to Safety immediately.

Forward-Facing Dash Camera Consent (Samsara). I acknowledge that company trucks are equipped with a Samsara forward-facing dash camera that records the road ahead. No inward-facing camera and no biometric data are collected. Footage may be reviewed by Safety in connection with accidents, complaints, or DOT inspections.

How are you working with us?
All Pro Logistics drivers — both owner-operators and company drivers — work as 1099 independent contractors. You'll be filed on a W-9. No taxes withheld; you handle your own quarterly tax filings.
Owner-Operator (1099)
You own or lease your own truck. Revenue split applies (typically 88% to you, 12% to PLS dispatch).
Company Driver (1099)
You drive a truck owned/leased by Pro Logistics. Paid per mile as a 1099 contractor — you're responsible for your own taxes.

Final certification. Once you sign and submit, your application goes to Anna for safety review and Vlada for approval.
